<p>FLU CLINIC is 11am - 12pm</p>

<p>Seniors 65+ with Medicare Part B: Bring your card</p>

<p>If you have an HMO, it is preferred that you go to your own physician for a vaccination. &nbsp;All those receiving a vaccine must sign the form.</p>

<p>RABIES CLINIC is 7pm - 8pm</p>

<p>Cats and dogs alllowed, must be accompanied by adult.</p>

<p>Moonachie residents, all dogs MUST have 2016 Dog License, which will be available at the clinic.</p>

<p>$10 - spayed / neutered</p>

<p>$13 - not spayed / neutered</p>

<p>EXACT Cash or checks payable to &#39;Borough of Moonachie&#39; only</p>

<p>If you purchase a dog license at the Rabies Clinic, the license still expires January 31, 2017 and must be renewed January 2017.</p>
