Vital Records Fees Waived In Flood Affected Counties
The Missouri Department of Health and Senior Services (DHSS) has announced that fees will be waived for replacement of Missouri birth, death, marriage and divorce records for persons whose documents were lost or destroyed due to the recent flooding.
Lincoln County Health Department (LCHD) issues copies of birth certificates from 1920-present and death certificates from 1980-present. To obtain a copy of a birth or death certificate a person must present a document containing a photo, such as a driver’s license, passport, or school, work, or military identification. If an applicant does not have a photo ID, at least two alternate forms of identification must be presented. Certificates may be obtained in-person at LCHD, Monday through Friday from 8:00 AM until 4:30 PM.
Birth and death certificate application forms may also be printed from the LCHD website at http://www.lchdmo.org and mailed to 5 Health Department Drive, Troy, MO 63379.
Requests for vital records submitted by mail must be notarized by a notary public.
