COVID-19 Vaccine 
Polk County has recently received its initial supply of the Moderna COVID-19 Vaccine. Polk County Health and Human Services and partners have started the vaccination process this week and will be providing additional vaccinations by appointment to Phase 1A populations on Tuesday, December, 29th. For those interested in receiving the vaccine please see the below link to register your interest and to determine which Phase of vaccine distribution you are eligible for at this time.
