Fatal Crash Investigation 
On June 24, 2021, at approximately 5:54 AM the Middleton Police Department received a call reporting a single vehicle crash on eastbound USH 12 at the Greenway Boulevard offramp. Upon arrival, officers discovered the driver was deceased. There were no other injuries in the crash, and the name of the driver is being withheld pending notification of family.
The crash remains under investigation by the Middleton Police Department, the Dane County Sheriff’s Office, and the Dane County Medical Examiner’s Office. Middleton Police were also assisted by Middleton Fire, Middleton EMS, and Dane County Public Highways.
The exit to Greenway Boulevard was closed for about four hours and re-opened at about 10:00am.
