- Participants 3-64 years of age are welcome; there is a $20.00 fee for vaccination. Preservative free $30.00
- Participants 65 years or over must bring a Medicare Part B number or pay a fee.
- Participants 65 years and over who also have a HMO must go to their primary care physician.
