FUNDING UPDATE -Additional $250,000 Approved for COVID-19 Vaccine Incentive
A family of 4 can receive up to $800 in cash incentive
November 12, 2021-San Benito County: On November 9, 2021, the San Benito County Board of Supervisors authorized an additional $250,000 for the $200 Cash COVID-19 Vaccine Incentive Program.
San Benito County residents who get fully vaccinated
after August 9, 2021, are eligible for a
$200 cash incentive,
including minors. Additionally, qualified individuals can receive a $25 gift card after they receive their
first dose in the COVID-19 vaccination series. San Benito County residents, including minors, who receive a first dose (Moderna/Pfizer) or single dose of J&J after August 9, 2021, are eligible for the incentive.
Gift cards are available at San Benito County Public Health Services at 351 Tres Pinos Rd. Suite A-202, Hollister, CA 95023. Monday through Friday from 8:00 am to 5:00 pm.
Individuals must bring proof of
vaccination,
identity and
San Benito County residency or San Benito County government employment. These incentives are available to all San Benito County residents regardless of immigration status.
The following are approved verification documents:
Vaccination Verification:
-CDC Covid-19 Vaccination Record Card
- Documenting first dose received on or after August 10, 2021
-CDPH Digital Smart Health Card
- Documenting first dose received on or after August 10, 2021
Adult Verification:
-Government Issued Photo Identification
-County Employee Paystub/ID (Employed by County of San Benito Only)
If ID does not have San Benito County address, then one of the following with photo ID
- Utility Bill/ Insurance Statement
- Mortgage Bill
- Rental/Lease Agreement with Signatures
Minors under 18 Years Old Verification (must show parent or guardian/child relationship)
-Birth Certificate
- With Parent/Guardian Residency Documentation
Currently, the County is providing COVID-19 vaccines to individual 5 years and older.
