Narrative:
On October 13th, 2022, a combined team of detectives from the Ventura County Sheriff’s Office Moorpark Investigations Bureau, Pharmaceutical Crimes Unit, and the United States Drug Enforcement Administration served a search warrant in the 600 Block of N. Central Avenue in the City of Glendale.  Detectives arrested Jack Julfayan, 40 years old, for multiple counts of Forging Prescriptions, Obtaining Prescription Medication through Fraud, Identity Theft, and Burglary. 
 
The case began in mid-July when patrol deputies from the Moorpark Station responded to a local pharmacy regarding a suspect who purchased a large quantity of an opiate-based prescription medication using a forged prescription.  Deputies completed the initial investigation and the case was assumed by detectives from the Moorpark Investigations Bureau.  Using a variety of investigative methods, detectives identified the suspect as Jack Julfayan of Glendale.  Because of the nature of the case, detectives then sought assistance from detectives from the Pharmaceutical Crimes Unit, who in turn, involved agents from United States Drug Enforcement Administration.  Through the combined efforts of the three teams, detectives determined that Julfayan had committed similar offenses at multiple locations within Ventura County, and likely throughout southern California. 
 
The case culminated on the afternoon of October 13th, 2022, when the detective teams served a search warrant at Julfayan’s residence in Glendale and arrested him without incident. During the service of the warrant, detectives located evidence linking Julfayan to the crimes.  Julfayan was transported back to Ventura County and booked at the Ventura Main Jail.  He faces thirteen counts of Forging Prescriptions and Obtaining Prescription Medication Through Fraud, six counts of Identity Theft, and four counts of Burglary.  Julfayan’s Bail was set at $300,000.00.
