Members of the Cal OES/FEMA Recovery Team will be in our area on Sunday and Monday to survey the damage based on the submission of these forms by members of the public and local businesses. Please complete the form today. 

Submitting your form does not mean you will automatically qualify for future assistance. Your submission of the damage survey form can help ensure Contra Costa County is included in the Federal Major Disaster Declaration. If our County is part of the Major Disaster Declaration, the availability of resources and assistance will be far greater.
