Complete your Health History Interview before you come in for your donation. Go to https://cflexpress.giveblood.org/ and complete the same day as your appointment. Please note the health history questionnaire MUST be completed on the day you intend to donate. Otherwise, you will be required to retake the questionnaire on the day of your donation.
