Cook County is gathering information on storm-related damage from the recent storms that impacted our area between August 16–19, 2025.

Cook County Emergency Management and Regional Security (EMRS), in coordination with state and local agencies, is collecting reports of property damage to determine the scope of impact and explore potential avenues for assistance.

If your property is within Cook County, please complete the brief Damage Assessment Survey at the link below. The information you provide will help the County better understand local needs. Submitting this form does not guarantee county, state, or federal assistance; it is used to assess costs and determine whether assistance may be available
