City strongly urges storm-impacted residents to report damage as soon as possible
The City and County of Honolulu’s Department of Emergency Management (DEM) is strongly urging residents impacted by the Kona low storm to report any damage as soon as possible.
DEM has released an online form for O'ahu residents to self-report damage to their homes. The information provided will help county officials and City partners better understand the damage that occurred.

When submitting the form, please note that the purpose of this form is only to collect information that will help local officials and our non-profit partners understand the damage that occurred and impacts on the community.
The self-reporting tool is NOT an application for relief programs or services.
Please note:
- Do not use this form to report immediate, life-threatening needs. Call 9-1-1.
- Reporting damage is a voluntary activity and is not a substitute for reporting damage to your insurance agency, nor does it guarantee disaster relief assistance.
- Please submit one survey per affected household. If living with roommates, please submit one per roommate.
- This form is for damage that occurred on O‘ahu only.
- If you need financial or other assistance, call 2-1-1. For life-threatening needs, call 9-1-1.
