Damage Assessment Survey from Storms on July 27, 2026 by the Illinois Office of Homeland Security

Illinois Initial Damage Assessment for Possible Disaster Relief Assistance

In partnership with the Illinois Emergency Management Agency Office of Homeland Security, the counties are compiling information related to damages that occurred from the thunderstorms and tornadoes on July 27, 2026.  Residents and businesses affected by these storms can submit an initial damage estimate to help determine if the counties can pursue disaster relief assistance on behalf of impacted property owners.

Reporting damages in this survey is voluntary.  The form takes approximately 10-15 minutes to complete.  This survey is not a substitute for reporting damage to your insurance agency and does not guarantee financial assistance. This survey is collecting data for needs that are not life-threatening.  To report dangerous conditions or a life-threatening emergency, call 9-1-1.

The survey closes Wednesday, Aug. 5, 2026. Respond to the survey by visiting the Illinois Residential Self-Assessment Survey online or downloading the link with the QR Code below:

Note: The survey is available in English, Spanish, Polish, Ukrainian or Mandarin by selecting the preferred language at the top of the survey.
