Emergency Information Sheet

Michiana Residents,

It has been a few years since I have sent out an updated emergency information sheet.

I would encourage everyone to fill this out, so that we can have accurate and up to date information in the event an issue occurs at your residence.

If you have multiple homes in Michiana, please fill it out for each home.

If you would prefer to fill a paper version out, please email me and I will send it over.

As always thank you for your support,

Ryan Layman, Chief of Police
