(Update) Damage Assessment Information

DAMAGE ASSESSMENT FORMS
Residents who received water damage to their properties from the storm on Friday, June 25 and Saturday, June 26, 2021 should fill out the updated Damage Assessment Form from the State of Michigan due to the State of Emergency declaration.
 
Completed forms should be turned into the City Clerk's office by 4 p.m. on Friday, July 2, 2021:

  • Email: [email protected].
  • In person at the City Clerk's office.
  • Placed in the drop box located in front of the Robert E. Novitke Municipal Building.
The State has provided the Damage Assessment Form for privately owned residences and businesses. The purpose of the reporting form is to collect information only reporting to the State pursuant to Public Act 390 of 1976 and is not mechanism for any other type of reimbursement or financial assistance. This is not a damage claim form pursuant to Public Act 222 of 2001.

It is highly suggested that residents take photos of the damage (including structural damage). An itemized loss list should be completed as well. It is possible that photo documentation and an itemized loss list will be required in the future.
