You can’t always predict when a medical emergency or serious illness will happen or the impact it will have. However, you and your loved ones can prepare for these events by sharing your wishes for care.
The Franklin Township Mayor’s Wellness Campaign and its partners are sponsoring the Five Wishes Workshops, the nation’s only national advance care planning program for more than 25 years.
This FREE program will help you complete the Five Wishes guide, so you can easily share your wishes about the type of care you would want to receive in a medical emergency or serious illness. Let us help you and your loved ones start the conversation about what in life matters most.
